The Extra Help program, also known as the Low-Income Subsidy (LIS), is designed to assist Medicare beneficiaries in reducing the cost of their prescription drugs. It is offered through the Social Security Administration and is one of the most straightforward ways to save money if you are enrolled in Medicare Part D.
Who Qualifies:
Must have Medicare Part A and/or Part B AND Medicare Part D
Limited income (approximately $22,000/year for individuals or $30,000/year for couples)
Limited assets (below $17,220 for individuals or $34,360 for couples in 2025)
If you are receiving Medicaid, Supplemental Security Income (SSI), or are part of a Medicare Savings Program, you automatically qualify.
What Extra Help Covers:
Up to 100% of your monthly Medicare Part D premium (for benchmark plans)
No or low deductibles for prescriptions
Significantly reduced copays (as low as $1.55–$10.35 in 2025)
Protection from the Part D coverage gap (donut hole)

In person: Visit your local Social Security office
Applications are typically processed quickly, and you will receive a determination by mail. If you are approved, your drug plan will be adjusted accordingly.
