Aging with Intention

The Community Care for the Elderly (CCE) Program provides community-based services organized in a continuum of care to help functionally impaired elders live in the least restrictive yet most cost-effective environment suitable to their needs.

Eligibility Requirements: Individuals must be age 60 or older and functionally impaired, as determined by an initial comprehensive assessment and annual reassessments.

Local appointed public guardians to provide guardianship services to persons who do not have adequate income or assets to afford a private guardian and there are no willing family or friends to serve.
